38341Loạt ca tắc nghẽn đường thở lớn phát hiện bằng hô hấp ký
Đường thở lớn gồm những đường dẫn khí có kích thước từ 2 mm trở lên. Tắc nghẽn đường thở lớn (TN-ĐTL [large airway obstruction]) đôi khi không có triệu chứng nhưng cũng có thể có các triệu chứng như khò khè, khó thở. Vì vậy, TN-ĐTL có thể bị chẩn đoán nhầm với bệnh lý đường thở nhỏ. Phương thức điều trị khác nhau theo từng nguyên nhân. Do vậy, chẩn đoán cần sớm và chính xác. Hô hấp ký là một trong những phương tiện ban đầu để chẩn đoán rối loạn này. Đường thở lớn được chia thành hai đoạn: trong và ngoài lồng ngực. Do sự khác nhau về áp suất trong và ngoài lồng ngực ở thì hít vào và thở ra nên sự tắc nghẽn sẽ biểu hiện khác nhau trên hô hấp ký trong các thì hô hấp tuỳ theo vị trí tắc nghẽn. Chúng tôi xin báo cáo năm trường hợp lâm sàng TN-ĐTL phát hiện bằng hô hấp ký. Trong đó, hai trường hợp nghi ngờ là hen, một trường hợpxảy ra đồng thời với bệnh phổi tắc nghẽn mạn tính (chronic obstructive pulmonary disease COPD), một trường hợp được chẩn đoán là hen khó trị, một trường hợp không có triệu chứng.
